HLP Homeless Health: London Hospital Discharge Housing Options Directory

Check out the new London Hospital Discharge Housing Options Directory on FutureNHS.

We understand that it can be complicated to refer individuals experiencing (or at risk of) homelessness to local authorities for long- or short-term accommodation. To help provide clarity around who to contact and local processes, we have worked with London Councils to develop The London Hospital Discharge Housing Options Directory. This centralised resource includes guidance and local authority contact information for boroughs in London. We hope it will allow for greater transparency and partnership working between frontline teams involved in out-of-hospital care and Local Authority housing teams. 

To access and use the directory:

  1. Create a FuturesNHS Account here
  2. Head to the London Hospital Discharge Housing Directory page on the FuturesNHS website
  3. Watch this short video to learn how to navigate the directory, for further guidance see the FAQ attached

Our homeless health Programme Manager, JJ Nadicksbernd discusses these topics with Tim Gray, an expert housing consultant, and David Woodley, a care navigator from the homelessness charity, Groundswell.
